Register of Run 4 Vision 2016 (June 18, 2016)

To register, follow these simple steps:

  1. Fill out and submit the form below
  2. If you would like to register additional family members, click "Add family members" and fill out the remainder of the form
  3. We will send you a confirmation email with the total registration fee (you and your family members)
  4. Bring a check to the event for your registration fee made out to "Foundation Fighting Blindness", to ensure that your registration fee is tax deductible

Register Family Members - do not include yourself!

Please provide a comma separated list of all adult names with t-shirt size (sm, md, lg, xlg, xxlg) (e.g., Bill Smith(lg), Mike Smith(xlg).

Please provide a comma separated list of all children names. T-shirts are available for children under 15 for $10.00 each, if you would like to purchase, add t-shirt size (sm, md, lg, xlg, xxlg) to the list as follows.(e.g., Billy Smith(sm), Jenny Smith(md), Liz Smith(md).
